Advocate Good Samaritan Hospital
Dear Metabolic and Bariatric Surgery Patient,

Your bariatric surgery program uses an Enhanced Recovery Pathway to make your pre- and post-operative experience predictable, safe and more pleasant. Importantly, the goals of Enhanced Recovery Pathway include using a variety of medications to treat pain and nausea while minimizing the use of narcotic pain medicine. Other components of the pathway may help reduce complications and can help you have a quicker recovery and return to normal activity.

This quality improvement project is being conducted to monitor your satisfaction with four stages of your care: preoperative, intraoperative, postoperative, and after discharge. This survey should take no more than approximately 5-10 minutes to complete. There are no risks involved in participating in this quality improvement project.

Your answers to the questions provided here will remain strictly confidential and your individual results will not be shared directly with your surgeon or the center who provided your care. Your decision to participate is entirely voluntary and refusal to take part in or withdrawing from the quality improvement project will involve no penalty toward you. In the future, the answers you provide may be published in a research paper, but will not identify you in any way.

If you have questions at any time, feel free to contact the principal investigator, Stacy Brethauer, MD, FACS, FASMBS, at the E-mail of: brethas@ccf.org.

Thank you in advance for your participation.
